- Chaplains, social workers and nurses available to meet physical, emotional, spiritual and social needs
- Planning and involving family regarding health goals and care
- Nurse practitioner on staff for consultation
- Pain medication management with consulting pharmacist
- Ongoing PT, OT and ST if needed
Program goals
- Support the patient and family through this difficult time.
- Soften the transition to hospice for patients and family members who are having difficulty with coming to terms with the prognosis
- Promote the utilization of palliative and hospice care
- Reduce hospital re-admissions and emergency room visits
- Reduce the number of unnecessary and costly interventions
