Program content

  • Chaplains, social workers and nurses available to meet physical, emotional, spiritual and social needs
  • Planning and involving family regarding health goals and care
  • Nurse practitioner on staff for consultation
  • Pain medication management with consulting pharmacist
  • Ongoing PT, OT and ST if needed

Program goals

  • Support the patient and family through this difficult time.
  • Soften the transition to hospice for patients and family members who are having difficulty with coming to terms with the prognosis
  • Promote the utilization of palliative and hospice care
  • Reduce hospital re-admissions and emergency room visits
  • Reduce the number of unnecessary and costly interventions
