Providers – How to Qualify

Qualifications will vary depending on pay source.

State Funded Programs

100% paid by the Department on Aging and Disability Services (DADS) – income and resources may not exceed specified limits for these programs. Call your local DADS office to apply for these programs.

Primary Home Care – In-home attendant services to people 21 or older with an unmet need for home management and personal care according to a practitioner’s statement of medical need. Must be eligible for Medicaid; meet specified functional assessment score and have a functional limitation with at least one personal care task based on a medical condition.

Community Attendant Services (CAS) – In-home attendant services to people who need help with personal care tasks whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner’s statement of medical need.

Family Care (FC) – In-home attendant services to people who may need help with personal care tasks whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner’s statement of medical need.

Community Based Alternatives (CBA) – Services to people who are older or who have disabilities as a cost effective alternative to a nursing home. You must be 21 or older; meet medical necessity criteria for nursing home care and be at risk for being placed ina nursing home; be eligible for Medicaid in the community including:

  • Being eligible for Supplemental Security Income
  • Being eligible for Medical Assistance Only protected status, or
  • Meeting the income and resource requirements for Medicaid benefits in a mursing home (spousal impoverishment provisions apply).
  • You cannot be enrolled in another waiver program.

HMO – Our agency currently has contracts with Amerigroup and Molina, the criteria for these programs is the same as it is for PHC and CBA. You must enroll with one of these HMOs in areas where DADS does not administer the PHC and CBA programs.
